We are here for you, offering extended hours that include evenings, Saturdays, and after hour emergency service. We accept many insurance plans, including some HMOs and PPOs. Contact your insurance company for verification of coverage. We offer flexible payment options, with low and even no-interest payment plans.

1. New Patient Information Form
To expedite the check-in process, please fill out the New Patient Information Form. By pre-submitting this information, you can avoid having to fill it out on your arrival!

2. Refraction Exam Acknowledgement
Insurance companies have informed us that we must separate the part of your eye examination that is called the “refraction” from the medical part of the examination. Please complete our Refraction Exam Acknowledgement.

3. Lifestyle Questionnaire
To better understand your eyecare needs, we ask that you download, print and complete our Patient Lifestyle Questionnaire. (This is a PDF document; you will need the free Acrobat Reader to view and print it.)
