To expedite the check-in process, please fill out the Patient Information Form. By pre-submitting this information, you can avoid having to fill it out on your arrival!

2. Medical Exam and Refraction
Insurance companies have informed us that we must separate the part of your eye examination that is called the “refraction” from the medical part of the examination. Please read this important information.
