Do Blue-light Filtering Lenses Work?
Blue-light filtering lenses prevent ultraviolet and some short-wavelength visible light from reaching the eyes. They supposedly provide retinal protection, reduce eye strain, and improve sleep quality. While many people swear by them, a review of multiple well-controlled studies showed little to no effect on visual acuity, computer eye strain, contrast sensitivity, color discrimination, discomfort glare, macular health, serum melatonin levels, sleep quality or overall patient visual satisfaction. However, as there were no noted harmful effects, wearing blue-light filtering lenses is an individual choice.
How to Choose the Best Artificial Tear:
There are two forms of dry eye disease: aqueous-deficient (your body does not produce enough tears) and evaporative (a deficient oily layer of the tear film causing tears to evaporate more quickly).
For aqueous deficiency, use thinner drops or those which are “hypotonic” or “hyposmolar.” For evaporative, use lubricants with lipid or oil-based components. Avoid “store brands” which often contain irritating preservatives (many have been recently recalled by the FDA). Also avoid those that are marketed to “get the red out” which contain vasoconstrictors to shrink blood vessels and can actually worsen symptoms.
Preservative-free drops are best for frequent use. They come in single-use and multi-dose bottles which use sophisticated technology built into the tip of the bottle to filter out bacteria. Either are fine assuming reputable providers.
Gel-based drops are thicker than liquid and ointments are the thickest. The thicker the lubricant, the longer it lasts, but the more visual blur when instilled. Most patients tend to use the liquid drops during the day and the gels or ointments at bedtime.
If you use drops while wearing contact lenses, use preservative-free lubricants that are “safe/designed for contact lens wear.” For other drops, take out your lenses first and then wait 5-10 minutes before re-inserting them.
No single brand works best for everyone. You may need to try different types or a combination before you find what works best for you. Considerations are cost, convenience and environmental impact.
Moving Dots in a Clear Blue Sky
If you’ve noticed moving dots in your vision when looking up at a bright blue sky, rest assured they are perfectly normal. They are caused by white blood cells passing through retinal blood vessels. Red blood cells, which make up more than 90 percent of your blood, absorb blue light. The larger white blood cells allow blue light to pass through to the retinal receptors, so as they pass through the vessels they cause the dots, which look like little worms moving around in your vision. The speed of the dots vary with your pulse and are perfectly normal. This “blue field entopic phenomenon” should not be confused with floaters which are due to changes in the vitreous and can be a sign of serious disease.
